Sheffield referrals

In Sheffield we accept referrals from individuals aged 14 plus, this may be done directly using the referral form at the bottom of this page.

OUR REFERRAL PROCESS

Referrals can be completed using the self-referral form at the bottom of this page. The referral form asks you for a bit of information about the difficulties you are experiencing. Once this is submitted someone from our clinical team will look through the information. We may contact you to discuss the referral before offering you an appointment for an assessment.

At an assessment someone from the team will meet with you to discuss what has brought you here and discuss what may be helpful. This is a collaboration between us and you to help you to find the most suitable source of support at that time.

At the end of the assessment we can discuss together whether we feel SYEDA is the most appropriate service for you, and if this is the case, you will be offered the most appropriate support.

Please note at the point of referral and/or assessment we may recommend that you are referred to an alternative service should we feel that your referral doesn’t meet our criteria, and we feel that an alternative service would be best to support your needs.

We also work closely with GPs and therefore we have a duty of care to contact your GP following referral and/or assessment to monitor your physical health.

We are a confidential service, but should we feel that you are at immediate risk of harm, we would need to break this confidentiality and contact the relevant organisation.
